Thank you for allowing us to be a part of your life during these difficult and trying circumstances. We want to do everything we can to best serve you. Please keep the following things in mind as you prepare for counseling.
Confidentiality
Here at Cord of 3, we take your confidentiality very seriously. Make sure that the URL toolbar has the secure padlock in it as you complete these papers. We work hard to ensure all your information is confidential.
Scheduling Your Appointment
In order to schedule your appointment, we must first get some basic information from you over the phone. Please call the office at 912-282-0992 and speak to one of our receptionists. They will ensure that we have what information we need in order for you to proceed.
Keep in Mind
- Your first appointment will be with an intake therapist. The purpose of this session is to gather the pertinent information we need in order to schedule you with the best counselor for your concerns. Not all therapists are certified in the same areas which is why we work to make the best fit between you and your counselor.
- We cannot bill private insurance companies (because we are a Faith based organization). However, we will work with you to make sure that the cost of counseling is no barrier to you. If you need some assistance with the financial side of counseling, please contact Leslie Lanier at 912-282-0992 to schedule a Family Fund appointment and go ahead and complete the application at this link:
FF Application - We bill your account at the time of services. Our self pay rate is $95 per session. For all self pay, we bill for those services at the time of counseling. We do ask for your credit card information up front so that we can bill at the time of services so please be prepared for that to be withdrawn from your account. If you need to set up a payment plan, our office will work with you.
- We do not have a receptionist at all of our counseling sites. Your therapist will be coordinating all of your needs at your counseling session. Upon your arrival, your therapist may still be completing a session with another family. If that is the case, please wait a few minutes and your therapist will be right with you. If you have any questions upon arrival at your appointment, feel free to contact our secretary at 912-282-0992 by calling or texting. Paperwork
We try to keep the paperwork to a minimum. With that being said, we do have a few forms for you to complete. Please take the time to complete these forms in their entirety. If you have left any undone or any sections blank, it will delay the scheduling of your counseling services. If your last appointment with us was more than one year ago, we will need to go back through the intake process and we will ask you to complete another set of paperwork.
Use the form below to send in your paperwork. Please complete all fields and then follow the instructions in order to submit your forms. You will see a "Submit" button appear as you begin completing the forms. Once you have completed all the forms to the best of your ability, submit them. Keep in mind: these forms are CONFIDENTIAL, only you will be able to see the information you put on the form.
You will be asked to provide your email address as part of submitting the form. Once you submit, you will then receive a secure email with your signed forms. If you do not receive that email, then your forms did not get submitted.

There are occasions when we need to talk with someone outside of the counseling session (your doctor, school teacher, lawyer). In order for us to talk with anyone, we must have a Release of Information signed by you first. If you foresee the need for us to talk with anyone other than you, the "Identified Patient", then go ahead and print out this form, fill it out, and provide it to your intake counselor. Cord of Three is dedicated to providing you with excellent services in a comfortable, safe, and private environment. If at any point, you feel that your rights as a client or your safety have been compromised, you may share your concerns with Cord of Three by calling 912-282-0992 and asking to speak with our Executive Director or the Chair of our Board. All concerns are kept confidential.
